HIV/AIDS
The continuum of care for HIV/AIDS services at Consejo includes Medical Case Management as well Partner Counseling and Referral Services to stop new infections among gay and bisexual men, transsexuals, men who have sex with men and heterosexual Latinas. Medical Case Management
Consejo’s HIV/AIDS Medical Case Management Program provide clients with a range of services including support in the meeting of basic human needs such as food and housing. Clients likewise access individual and group therapy, HIV Partner Counseling and Referral Services, behavioral health services and medication management. Additionally, Consejo Medical Case Managers advocate for clients while they access and applying for primary medical and dental care, HIV programming available through local health agencies as well as linkage to community resources as needed.
Vida Nueva
Provides highly intensive, ongoing, individualized HIV prevention counseling, emotional support and linkage to community resources. All activities address the relationship between HIV risks and substance abuse, other STD’s, mental health issues, language barriers and the many social and cultural factors that affect Latinos living with HIV and their partners, families and social networks.
